• 2013 MINOR RELEASE FORM

  • No minors will be permitted to compete unless they have one of the following with them at sign-up:
    • Both parents must be present to sign the release form, or
    • One absent parent must have signed AND NOTARIZED this form which the other parent must sign at sign-up
    • NO EXCEPTIONS.
